What are bitewing X-rays and what are they used for?
Bitewings, also known as interproximal X-rays, are a type of intraoral examination specifically designed to visualise the crowns of the posterior teeth — premolars and molars — in both the upper and lower arches in a single image.
The name "bitewing" derives from the small "bite wing" that the patient bites down on to stabilise the film or sensor during the examination.
This is the gold-standard radiographic examination for the early detection of interproximal caries — cavities that develop on the lateral surfaces of the teeth, in the contact areas between adjacent teeth.
These cavities are virtually impossible to identify with the naked eye during a conventional clinical examination.
At Clínica Dr. Paulo Cardozo in Benavente, we use digital bitewings that provide high-resolution images with significantly lower radiation doses.
In addition to detecting caries, bitewings are also used to assess the fit of existing restorations, check for subgingival calculus, and monitor the health of the alveolar bone crests — making them an important complement to the panoramic X-ray.

How bitewing X-rays work
The bitewing examination is quick, comfortable, and particularly useful during routine appointments. At Clínica Dr. Paulo Cardozo, we use digital intraoral sensors that provide high-resolution images with minimal radiation exposure for the patient.

Patient Preparation
The patient is asked to remove any metal objects from the head and neck area. A lead protective apron is fitted and the technician selects the appropriate digital sensors for the posterior tooth region to be examined.
Sensor Positioning with Bite Wing
The digital sensor, protected by a disposable cover, is placed inside the patient's mouth in the area of the posterior teeth. The patient gently bites on the wing of the holder, closing their teeth naturally, which stabilises the sensor in the ideal position to capture the image.
Interproximal Image Capture
The X-ray tube is positioned at the correct angle and the exposure is taken. The procedure is repeated for the different areas — molars and premolars on both sides — typically producing four images in total. Each exposure is painless and lasts less than one second.
Immediate In-Surgery Analysis
The digital images appear instantly on the monitor. The dentist analyses the interproximal areas of the teeth, checking for the presence of decay, changes to restorations, tartar, or bone crest loss. The results are discussed with the patient in real time.

What are bitewing radiographs?
These are interproximal X-rays that show the crowns of the upper and lower posterior teeth, ideal for detecting cavities between teeth.
Is the examination painful or uncomfortable?
No. Placing the sensor is straightforward, and the capture takes less than a second. The slight contact of the sensor in the mouth is well tolerated.
How many radiographs are taken?
Four images are usually taken: two of the molar area and two of the premolar area, covering all posterior teeth.
How often should I have bitewing radiographs?
We recommend every six or twelve months, depending on the patient's risk of tooth decay. The dentist will advise on the appropriate frequency.
Are bitewing radiographs safe for children?
Yes. The radiation dose is very low, and the examination is carried out with all appropriate protection for the child's age, including a lead apron.
What is the difference between bitewing and periapical radiographs?
Bitewing X-rays show the crowns of the posterior teeth. A periapical X-ray shows the complete tooth, from the crown to the root tip.
Can cavities be detected with the naked eye?
Not always. Interproximal cavities, which form between teeth, are practically invisible to the naked eye and are only detected by bitewing X-rays.
Does the examination require any special preparation?
No. It is simply necessary to remove any metal objects from the head area. No fasting or any other prior preparation is required for the examination.
